Call and talk with Nancy. She will set up your first visit with you, get your insurance information and send out your initial paperwork so you can complete this on your own.

Our first available appointment is at 6 a.m. Our last appointment ends at 5 p.m.

If you are in acute musculoskeletal pain and need to been seen immediately, please call us. We are frequently able to work you into our schedule.

Evaluations are performed in a private room. Future visits are either in a private room and/or open area based on treatment needs. Your first appointment will include a thorough medical history and evaluation of your problem; as well as treatment, patient education and instruction.

 

 

What to expect on your first visit:

Plan on spending up to 60 minutes at your first visit. Please arrive 15 minutes before your scheduled appointment time so you can complete the patient paperwork. If the paperwork is complete in advance, plan to arrive five minutes prior to your appointment.

Bring:

1. Paperwork
2. Insurance care
3. Current doctor prescription (if required)
4. Claim number or pre-authorization form (If required)
5. Co-pay (if applicable) – check or cash, sorry no credit cards
6. Comfortable clothing

• For patients with low back or leg problems please bring shorts and a t-shirt. Stretch pants may suffice for future appointments. Evaluation is performed in a private room.
